Photo Release Form for Minor/Children

  • Liability Release and Parental Consent Form In consideration of the acceptance of my application for the above program, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of participation in said event. This release is intended to discharge in advance KID INDIA , its officials, officers, employees, volunteers and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some recreational activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees. Parental Consent (Complete if applicant is under 18) I give consent for my child.

    Hereby authorize KID INDIA to publish the photographs taken of me and/or the undersigned minor children, and our names, for use in the Rock Solid website and for display in the facility. I release KID INDIA from any expectation of confidentiality for the undersigned minor children and myself and attest that I am the parent or legal guardian of the children listed below and that I have the authority to authorized KID INDIA to use their photographs and names. I acknowledge that since participation in publications and websites produced by KID INDIA is voluntary, neither the minor children nor I will receive financial compensation. I further agree that participation in any publication and website produced by KID INDIA confers no rights of ownership whatsoever. I release KID INDIA , its contractors and its employees from liability for any claims by me or any third party in connection with my participation or the participation of the undersigned minor children.

  • Names and Ages of Minor Children:

  • In and Out For your child's safety, KID INDIA requires that children be signed in and out of the program by a parent or person listed on the registration form. Please do not drop off your child outside the facility.
    Children's Code of Conduct Children must adhere to the procedures set by Kid India staff.
    Children are expected to be responsible for their actions. KID INDIA will not be responsible for lost, stolen, or damages to items brought

  • 1. Fee is due every 30th / 31st of the every month for the following / coming month

    3. Child miss a day fee will be charged for the assign day. Federal holiday will be close too, but have to pay fee for that day.

    4. Food, snacks, diaper, formula from HOME only. We provide only care & education

    5. Fever, vomiting , dehire keep the child at home. If child has fever, vomiting,
    diarrhea at the care place will inform you, to pick up the child immediately

    6. To quit Kid India 1 month written notice required

    7. In case of long break you still have to pay fee or quit & rejoin again as with new enrollment fee [ if spot is available ]

    8. We follow MONTESSORI method of teaching. Everyday one special class which is extra curricular to nourish them through DANCE / YOGA / PAINTING /MUSIC/ LANGUAGE [NO extra charges for that]

    9. Extra pair of clothing & shoes deposit with enrollment. When dirty take them home to wash / clean [ generally on FRIDAY’S]
    10. B’day celebrations are allowed. Inform us at least a week before, to arrange schedule of the SPECIAL day.

    11. No detail / discussions with teachers/ staff while dropping or picking. Any discussions just Whatsapp /Message your concerns, will reply / call back
    ASAP. Remember we are busy taking care kids & nothing is important than your child

    12. Child progress review thrice a year. Dec30, April30, Aug30 will share progress with pictures, videos & other hands on activities.

    13 Your suggestions are welcome but only in writing not verbal. We have to work as a team to raise a good HUMAN BEING.

    • No child shall be spanked or subjected to any other form of physical punishment.
    • No child shall be subjected to any discipline, which is severe, humiliating, frightening, or
      any kind of discipline associated with depriving a child of necessary nutrition, rest, and
      toiling.
    • Discipline strategies used to accomplish desirable behavior include:
      • Positive reinforcement: Focusing on things children are doing right, rather than on
        negative behaviors.
      • Redirection: Distracting the child’s attention from the undesired behavior to something
        else.
      • For stalling (Proactive): Anticipating children’s actions before they happen. We can then
        intervene and stop a behavior form occurring (looking for warning signs that an action might be
        about to happen).
      • Modeling appropriate behavior. Showing children how to act by example.
      • Positive phrasing: Putting comments in a positive rather than a negative phrase
      • We can teach the children to resolve their own disputes by taking advantage of
        opportunities that arise in which we can guide them to peaceful cooperation. Older children are
        encouraged to help the younger children.
        If a child demonstrates physical acts of violence, disobedience and negative comments, the
        following actions will be taken:
    • We will immediately intervene.
    • We will assess the needs of the child being victimized to determine whether he or she requires any special attention (medical or otherwise).
    • The child aggressor will immediately be cautioned and directed to make amends (to apologize or share, for example).
      • If the inappropriate behavior continues, the disobedient child may be subject to:
        1 A time out; or 2 Being physically removed from the immediate vicinity of the incident;
      • 3 Being limited in partaking in the immediate activity or related future activities.
        if a dispute over a toy is the source of the incident, the toy may be temporarily removed.
    • The parents of an aggressive or disobedient child victim will be notified of an incident so that
      the parents may address the incident accordingly.
